The active ingredient present in Tegrital CR tablets is Carbamazepine. Each tablet of contains 200 mg, 300 mg or 400 mg of Carbamazepine.
Uses of Generic Tegretol XR® (Tegrital CR Tablets by Novartis)
This medicine is used to treat seizures and nerve pain such as trigeminal neuralgia and diabetic neuropathy as well as to treat bipolar disorder. It is also used off-label for a variety of indications, including attention-deficit hyperactivity disorder (ADHD), schizophrenia, phantom limb syndrome, complex regional pain syndrome, paroxysmal extreme pain disorder, neuromyotonia, intermittent explosive disorder, borderline personality disorder and post-traumatic.
The medicine belongs to a group of drugs called anticonvulsants that works by decreasing nerve impulses, which causes seizure and pain.
Tegrital CR Tablets (Generic Tegretol) - Dosage
The recommended adult Oral dosage of Generic Tegretol (Tegrital CR Tablets) for adults is given below:
Epilepsy
Adults and children over 12 years of age:
Initial: 200 mg twice a day.Increase the dosage at weekly intervals by adding up to 200 mg/day using a twice a day regimen until the optimal therapeutic response is obtained. Dosage generally should not exceed 1000 mg daily in children 12 to 15 years of age, and 1200 mg daily in patients above 15 years of age.
Maintenance: Adjust dosage to the minimum effective level, usually 800 to 1200 mg daily.
Trigeminal Neuralgia
Adult Dosage:
Initial: On the first day, 100 mg twice a day for a total daily dose of 200 mg. This daily dose may be increased by up to 200 mg/day using increments of 100 mg every 12 hours, only as needed to achieve freedom from pain. Do not exceed 1200 mg daily.
Maintenance: Control of pain can be maintained in most patients with 400 to 800 mg daily. However, some patients may be maintained on as little as 200 mg daily, while others may require as much as 1200 mg daily.
Adult Dosage for Prophylaxis Of Bipolar Disorder
Initial: On the first day, 400 mg/day in divided doses (200 mg twice a day). The daily dose may be increased gradually. The maintenence dosage is 400-600 mg/day in divided doses. Dosage must not exceed 1600 mg daily.
How to Use Tegrital CR Tablets (Generic Tegretol)
Take Tegrital CR tablets exactly as it was prescribed for you. Do not take the medication in larger amounts, or take it for longer or shorter than recommended by your doctor. Your doctor may occasionally change your dose to make sure you get the best results from this medication. It may take up to 4 weeks of using this medicine before your symptoms improve. Do not stop taking the medication even if you feel better. You may have increased seizures or unpleasant side effects if you stop using it suddenly. Call your doctor promptly if this medicine does not seem to be working as well in preventing your seizures.
Do not crush, chew, or break the tablet - swallow it whole. Breaking the tablet would cause too much of the drug to be released at one time.
This medicine can lower the blood cells that help your body fight infections. This can make it easier for you to bleed from an injury or get sick from being around others who are ill. Your blood will need to be tested on a regular basis to be sure your blood cell count does not get too low. Do not miss any scheduled appointments. Your doctor may recommend having your eyes checked regularly while you are taking this medicine.
Carry an ID card or wear a medical alert bracelet stating that you are taking this medicine, in case of emergency. Any doctor, dentist, or emergency medical care provider who treats you should know that you are taking this medicine. Grapefruit and grapefruit juice may interact with it and cause unwanted side effects. Do not change the amount of grapefruit products in your diet without first talking to your doctor.
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at the next regularly scheduled time. Do not take extra medicine to make up the missed dose.
Generic Tegretol (Tegrital CR Tablets) - Contraindications
Generic Tegretol tablets are contraindicated in patients with
- Hypersensitivity to Carbamazepine or any inactive ingredient present in Tegrital CR tablets
- Known sensitivity to any of the tricyclic compounds, such as amitriptyline, desipramine, imipramine, protriptyline, nortriptyline, etc.
- History of previous bone marrow depression
- Use with monoamine oxidase inhibitors is not recommended
- MAO inhibitors should be discontinued for a minimum of 14 days, or longer
- Coadministration of carbamazepine with nefazodone is contraindicated.
Carbamazepine Side effects
The most frequently observed Carbamazepine side effects, particularly during the initial phases of therapy, are dizziness, drowsiness, unsteadiness, nausea, and vomiting. To minimize the possibility of such adverse reactions, therapy should be started at the low dosage recommended.
The most severe adverse reactions have been observed in the hemopoietic system and skin, the liver, and the cardiovascular system.
Get emergency medical help if you have any signs of an allergic reaction including: hives, difficulty breathing, and swelling of your face, lips, tongue, or throat.
Call your doctor at once if you have any new or worsening symptoms such as: mood or behavior changes, depression, anxiety, or if you feel agitated, hostile, restless, hyperactive (mentally or physically), or have thoughts about suicide or hurting yourself.
Call your doctor at once if you have any of these serious Generic Tegretol side effects:
• fever, sore throat, and headache with a severe blistering, peeling, and red skin rash
• pale skin, easy bruising or bleeding, unusual weakness
• white patches or sores inside your mouth or on your lips
• feeling short of breath, swelling of your ankles or feet
• nausea, stomach pain, loss of appetite, jaundice (yellowing of the skin or eyes)
• urinating less than usual
Less serious side effects of Generic Tegretol may include:
• feeling dizzy, drowsy, or unsteady
• vomiting, diarrhea, constipation, stomach pain
• confusion, headache, blurred vision
• ringing in your ears
• dry mouth, swollen tongue
• joint or muscle pain, leg cramps
Overdose symptoms may include shallow breathing, fast heartbeat, nausea, vomiting, urinating less or not at all, muscle twitches, restlessness, tremors, slurred speech, staggering walk, and feeling light-headed or fainting. This is not a complete list of side effects and others may occur. Tell your doctor about any unusual or bothersome side effect.
Tegrital Tablets (Generic Tegretol) - Storage Requirements
Tegrital Tablets (Generic Tegretol) are to be stored at room temperature (15°C to 30°C). Store away from heat, moisture, and light. Keep away from children and pets.
Warnings & Precautions When Undergoing Treatment With Generic Tegretol
You should not use Generic Tegretol (Tegrital CR Tablets) if you are allergic to carbamazepine (the active ingredient) or to certain antidepressants such as amitriptyline (Elavil®), clomipramine (Anafranil®), imipramine (Tofranil®), and others. Do not use carbamazepine if you have used an MAO inhibitor such as isocarboxazid (Marplan®), phenelzine (Nardil®), rasagiline (Azilect®), selegiline (Eldepryl®, Emsam®), or tranylcypromine (Parnate®) within the past 14 days. You should not take carbamazepine if you have a history of bone marrow suppression or if you are also taking nefazodone.
Before taking this medicine, tell your doctor if you are allergic to any drugs, or if you have:
• a history of serious side effects from any drug
• liver or kidney disease
• porphyria
• heart disease or heart block
• lupus
• a history of mental illness
• glaucoma
If you have any of these conditions, you may need a dose adjustment or special tests to safely take this medicine.
You may have thoughts about suicide while taking this medication. Tell your doctor if you have new or worsening depression or suicidal thoughts during the first several months of treatment, or whenever your dose is changed. Your family or other caregivers should also be alert to changes in your mood or symptoms. Your doctor will need to check you at regular visits. Do not miss any scheduled appointments. Patients of Asian ancestry may have a higher risk of developing a rare but serious skin reaction to this medicine. Your doctor may recommend a blood test before you start the medication to determine your risk of this skin reaction.
This medicine can cause side effects that may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be awake and alert. Avoid drinking alcohol. It can increase some of the side effects of this medicine, and can also increase your risk of seizures. Avoid exposure to sunlight or artificial UV rays (sunlamps or tanning beds). This medicine can make your skin more sensitive to sunlight and sunburn may result. Use a sunscreen (minimum SPF 15) and wear protective clothing if you must be out in the sun. Cold or allergy medicine, narcotic pain medicine, sleeping pills, muscle relaxers, other seizure medicines, and medicine for depression or anxiety can add to sleepiness caused by it. Tell your doctor if you regularly use any of these other medicines.
Carbamazepine Tablets During Pregnancy
Carbamazepine has been classified by the US FDA as pregnancy Category D. There is positive evidence that Carbamazepine can cause fetal harm when administered to a pregnant woman. When treating or counseling women of childbearing potential, the prescribing doctor will wish to consider the benefits of therapy against the risks.
Epidemiological data shows that there could be an association between the use of Carbamazepine during pregnancy and congenital malformations, including spina bifida. Reports also suggest that Carbamazepine is possibly associated with developmental disorders and congenital anomalies (e.g.,craniofacial defects, cardiovascular malformations, and anomalies involving various body systems). Developmental delays based on neurobehavioral assessments have been reported.
If this drug is used during pregnancy, or if the patient becomes pregnant while taking this drug, the patient should be told of the potential hazard to the fetus.
This medicine may cause injury and even death to the unborn baby if you take it during the second and third trimesters of pregnancy. Do not use this medicine without your doctor's consent if you are pregnant.
Carbamazepine Tablets During Nursing
Carbamazepine (Generic Tegretol) and its epoxide metabolite are secreted in breast milk. The ratio of the concentration in breast milk to that in maternal plasma is about 0.4 for Carbamazepine and about 0.5 for the epoxide. The estimated doses given to the newborn during breastfeeding are in the range of 2 to 5mg daily for Carbamazepine and 1 to 2 mg daily for the epoxide.
Due to the potential for serious adverse effects in nursing infants from carbamazepine, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother. Do not use this medication without telling your doctor if you are breast-feeding a baby.
