Tadali Pocket (Generic Tadalafil 20 mg Tablets) belongs to a family of medications called phosphodiesterase-5 (PDE5) inhibitors is an oral medicine that is used for treating impotence (the inability to attain or maintain a penile erection).
Erection of the penis is caused by the filling of the penis with blood. Filling takes place because the blood vessels that bring blood to the penis increase in size and deliver more blood to the penis, and, at the same time, the blood vessels that take blood away from the penis decrease in size and remove less blood from the penis.
Sexual stimulation that leads to an erection causes the production and release of nitric oxide in the penis. The nitric oxide causes an enzyme, guanylate cyclase, to produce cyclic guanosine monophosphate (cGMP). It is cGMP that is primarily responsible for increasing and decreasing the size of blood vessels carrying blood to and from the penis, respectively, and causing an erection. When the cGMP is destroyed by another enzyme, phosphodiesterase-5, the blood vessels return to their normal size, blood leaves the penis, and the erection ends. Tadalafil prevents phosphodiesterase-5 from destroying cGMP so that cGMP stays around longer. The persistence of cGMP leads to a much longer engorgement of the penis with blood.

Tadali Pocket Contraindications

Generic Tadalafil Tablets (Tadali Pocket) - Warnings and Precautions

• Patients should not use Tadali Pocket if sex is inadvisable due to cardiovascular status.
• Use of Tadali Pocket tablets with alpha-blockers, antihypertensives or substantial amounts of alcohol (5 units or more) may lead to hypotension
• Tadalafil tablets (Tadali Pocket) is not recommended in combination with alpha-blockers for the treatment of BPH because efficacy of the combination has not been adequately studied and because of the risk of blood pressure lowering. Caution is advised when Tadali Pocket is used as a treatment for ED in men taking alpha-blockers.
• Patients must seek emergency treatment if an erection lasts for more than 4 hours. Use Tadali Pocket tablets with caution in patients predisposed to priapism.
• Patients should stop Tadali Pocket tablets and seek medical care if a sudden loss of vision occurs in one or both eyes, which could be a sign of non-arteritic anterior ischemic optic neuropathy (NAION). Tadali Pocket should be used with caution, and only when the anticipated benefits outweigh the risks, in patients with a history of NAION. Patients with a "crowded" optic disc may also be at an increased risk of NAION.
• Patients should stop Tadali Pocket tablets and seek immediate medical attention in the event of sudden decrease or loss of hearing.
• Before starting treatment with Generic Tadalafil tablets for Benign Prostatic Hyperplasia (BPH), consideration should be given to other urological conditions that may cause similar symptoms.
• Pulmonary vasodilators may significantly worsen the cardiovascular status of patients with pulmonary veno-occlusive disease (PVOD). Since there are no clinical data on administration of tadalafil to patients with veno-occlusive disease, administration of Tadalafil to such patients is not recommended.
• Concomitant PDE5 inhibitors: Avoid use with Tadali Pocket or other PDE5 inhibitors. The safety and efficacy of combinations of Tadalafil and other PDE5 inhibitors or other treatments for erectile dysfunction have not been studied. The patients should be informed not to take Tadalafil in such combinations.
• Due to increased Tadalafil exposure (AUC), limited clinical experience and the lack of ability to influence clearance by dialysis, once-a-day dosing of Tadalafil is not recommended in patients with severe renal impairment.
• Caution should be exercised when prescribing Tadalafil to patients using potent CYP3A4 inhibitors (Ritonavir, Saquinavir, Ketoconazole, Itraconazole, And Erythromycin), as increased tadalafil exposure (AUC) has been observed if the medicinal products are combined.

Hence, doctors must inform patients that substantial consumption of alcohol (e.g., 5 units or more) in combination with Tadalafil tablets can increase the potential for orthostatic signs and symptoms, including increase in heart rate, decrease in standing blood pressure, dizziness, and headache.
