Warnings and Precautions
The warnings and precautions associated with Lamivudine (Generic Epivir and Generic Epivir HBV) therapy are:
Co-infected HIV-1/HBV Patients: Emergence of Lamivudine resistant HBV variants associated with lamivudine-containing antiretroviral regimens has been reported.
Emtricitabine-containing products should not be administered concomitantly with Lamivudine containing products.
Hepatic decompensation, some fatal, has occurred in HIV-1/HCV co-infected patients receiving interferon and Ribavirin based regimens.
Monitor for treatment associated toxicities. Discontinue Lamivir HBV and Lamivir as medically appropriate and consider dose reduction or discontinuation of interferon alfa, Ribavirin, or both.
Pancreatitis: Use with caution in pediatric patients with a history of pancreatitis or other significant risk factors for pancreatitis. Discontinue treatment as clinically appropriate.
Immune reconstitution syndrome and redistribution/accumulation of body fat have been reported in patients treated with combination antiretroviral therapy.
In patients with moderate to severe renal impairment, serum Lamivudine concentrations (AUC) are increased due to decreased renal clearance, therefore the dose should be reduced for patients with a creatinine clearance of less than 50 mL/minute.
Cases of lactic acidosis, usually associated with severe hepatomegaly and hepatic steatosis, have been reported with the use of combination nucleoside analogue therapy in patients with HIV.

Lamivudine Overdosage
Currently is no known antidote for Lamivudine. In case overdose occurs, the patient should be monitored, and standard supportive treatment utilized, as required.
Due to the fact that a negligible amount of Lamivudine was removed via (4-hour) hemodialysis, continuous ambulatory peritoneal dialysis, and automated peritoneal dialysis, it is not known if continuous hemodialysis would provide clinical benefit in a Lamivudine overdose event.
Lamivudine During Pregnancy
Lamivudine has been classified by the US FDA as Pregnancy Category C. There are no adequate and well-controlled trials of Lamivudine- HBV or Lamivudine in pregnant women.
Due to the fact that animal reproduction studies are not always predictive of human response, Lamivir- HBV and Lamivir should be used during pregnancy only if the potential benefits outweigh the potential risks to the fetus.
Do not use this medicine without your doctor's consent in case you are pregnant.
Nursing Considerations
Lamivudine is excreted in human milk. Since there exists a potential for serious adverse reactions in nursing infants, a decision should be made to discontinue Lamivir- HBV taking into consideration the importance of continued hepatitis B therapy to the mother and the known benefits of breastfeeding.
