The active ingredient present in Valclovir tablets is Valacyclovir Hydrochloride. Each film-coated tablet contains Valacyclovir Hydrochloride equivalent to 500 mg or 1000 mg of Valacyclovir.
Uses of Valclovir Tablets
Valclovir is a nucleoside analogue DNA polymerase inhibitor used for the treatment of Cold Sores (Herpes Labialis) and Genital Herpes.
This medicine is used for the treatment or suppression of genital herpes in immunocompetent individuals and for the suppression of recurrent genital herpes in HIV infected individuals.
Valclovir is also used to reduce the risk of transmission of genital herpes with the use of suppressive therapy. In Pediatric Patients, it is used to treat Cold Sores (Herpes Labialis) and Chickenpox.
Generic Valacyclovir Tablets (Valclovir) - Dosage
Generic Valacyclovir tablets (Valclovir) may be administered with or without food. The dose that you should take will depend on why your doctor has prescribed Valclovir for you. The recommended dose for various indications is given below:
Treatment of Cold Sores (Herpes Labialis) in Adults:
The recommended dosage of valacyclovir for the treatment of cold sores is 2 gm twice daily for 1 day, taken about 12 hours apart. Therapy should be initiated at the earliest symptom of a cold sore (e.g. tingling, itching, or burning).
Treatment of Genital Herpes in Adults:
- Initial Episodes: The recommended dosage of valacyclovir for treatment of initial genital herpes is 1 gm twice daily for 10 days. Therapy was most effective when administered within 48 hours of the onset of signs and symptoms.
- Recurrent Episodes: The recommended dosage of valacyclovir for the treatment of recurrent genital herpes is 500 mg twice daily for 3 days. Initiate treatment at the first sign or symptom of an episode.
- Suppressive Therapy: The recommended dosage of valacyclovir for chronic suppressive therapy of recurrent genital herpes is 1 gm once daily in patients with normal immune function. In patients with a history of nine or fewer recurrences per year, an alternative dose is 500 mg once daily.
In HIV-infected patients with CD4+ cell count more than 100 cells/mm3, the recommended dosage of valacyclovir for chronic suppressive therapy of recurrent genital herpes is 500 mg twice daily.
- Reduction of Transmission: The recommended dosage of valacyclovir for reduction of the transmission of genital herpes in patients with a history of nine or fewer recurrences per year is 500 mg once daily for the source partner.
Treatment of Herpes Zoster in Adults:
The recommended dosage of valacyclovir for the treatment of herpes zoster is 1 gm orally, three times daily for 7 days. Therapy should be initiated at the earliest sign or symptom of herpes zoster and is most effective when started within 48 hours of the onset of the herpes zoster rash.
Treatment of Cold Sores (Herpes Labialis) in Children:
The recommended dosage of valacyclovir for the treatment of cold sores in patients, 12 years of age and older, is 2 gm twice daily for 1 day, taken 12 hours apart. Therapy should be initiated at the earliest symptom of a cold sore (e.g. tingling, itching or burning).
Treatment of Chickenpox in Children:
The recommended dosage of valacyclovir for treatment of chickenpox in immunocompetent patients aged 2 to <18 years is 20 mg/kg, administered three times daily for 5 days. The total dose should not exceed 1 gm three times daily. Therapy should be initiated at the earliest sign or symptom.

Store Valclovir tablets (Generic Valacyclovir Hydrochloride) at controlled room temperature (59°F to 86°F, 15°C to 30°C) in a cool, dry place. Protect from light.
Valclovir Contraindications
Valclovir tablets are contraindicated in patients with a hypersensitivity to Valacyclovir Hydrochloride, Acyclovir, or any inactive ingredient present in Valclovir tablets.
Warnings and Precautions
- Thrombotic thrombocytopenic purpura/hemolytic uremic syndrome (TTP/HUS): Has occurred in patients with advanced HIV disease and in allogenic bone marrow transplant and renal transplant patients receiving 8 grams per day of VALTREX in clinical trials. Discontinue treatment with Valclovir in case clinical symptoms and laboratory findings consistent with TTP/HUS occur.
- Acute renal failure: May occur in elderly patients (with or without reduced renal function), patients with underlying renal disease who receive higher than recommended doses of Valclovir for their level of renal function, patients who receive concomitant nephrotoxic drugs, or inadequately hydrated patients. Use with caution in elderly patients and reduce dosage in patients with renal impairment.
- Central nervous system adverse reactions (e.g., agitation, hallucinations, confusion, and encephalopathy): May occur in elderly patients (with or without reduced renal function) and in patients with underlying renal disease who receive higher than recommended doses of Valclovir for their level of renal function. Use with caution in elderly patients and reduce dosage in patients with renal impairment.
- Care must be taken to ensure adequate fluid intake in patients who are at risk of dehydration particularly in geriatric patients.
- The safety and efficacy of Valacyclovir Hydrochloride have not been established for the treatment of disseminated herpes zoster.
- The safety and efficacy of valacyclovir hydrochloride have not been established in immunocompromised patients other than for the suppression of ano-genital herpes in HIV-infected patients. The safety and efficacy of valacyclovir hydrocholirde for the suppression of recurrent ano-genital herpes in patients with advanced HIV disease (CD4 cell counts less than 100 cells/mm3) have not been established.
- Patients should be informed that Valacyclovir Hydrochloride is not a cure for genital herpes. Safer sex practices should be used in combination with suppressive therapy.
Generic Valacyclovir tablets alone should not be used for reducing the risk of transmitting genital herpes. Because genital herpes is a sexually transmitted infection, patients should, in order to further reduce the risk of infecting partners, avoid contact with lesions, damaged skin/mucosa, and also avoid intercourse when lesions and/or symptoms are present.
Genital herpes is frequently transmitted in the absence of symptoms through asymptomatic viral shedding; therefore patients should be counselled to use safer sex practices.
- Central nervous system adverse reactions (e.g., agitation, hallucinations, confusion, and encephalopathy): May occur in both adult and pediatric (more than 12 years of age) patients (with or without reduced renal function) and in patients with underlying renal disease who receive higher than recommended doses of Generic Valacyclovir tablets for their level of renal function. Elderly patients are more likely to have central nervous system adverse reactions. Use with caution in elderly patients and reduce dosage in patients with renal impairment.
- Hepatic/Biliary/Pancreatic: Dose modification is not required in patients with mild or moderate cirrhosis (hepatic synthetic function maintained). Pharmacokinetic data in patients with advanced cirrhosis (impaired hepatic synthetic function and evidence of portal-systemic shunting) do not indicate the need for dosage adjustment. There are no data available on the use of higher doses of Generic Valacyclovir tablets (4 g or more per day) in patients with liver disease. Caution should therefore be exercised when administering higher doses of this medicine to these patients.
- Renal Renal insufficiency or acute renal failure has been observed in patients taking Valacyclovir Hydrochloride at the recommended dosage and/or with no previous renal conditions and may be associated with renal pain. Acyclovir, the active metabolite of valacyclovir, is eliminated by renal clearance, therefore the dose of Valclovir must be reduced in patients with renal impairment. Geriatric patients are likely to have reduced renal function and therefore the need for dose reduction must be considered in this group of patients.
- Pregnant Women: There are no adequate and well-controlled studies with either Acyclovir or Valacyclovir Hydrochloride in pregnant women.
Valclovir (Generic Valacyclovir Hydrochloride Tablets) Side Effects
The most common side effects reported in at least one indication by more than 10% of adult patients treated with Valacyclovir tablets (Valclovir) and more commonly than in patients treated with placebo are headache, nausea, and abdominal pain.
The only side effect occurring in more than 10% of pediatric patients less than 18 years of age was headache.
Side effects reported during the post-marketing period are:
- General: Facial edema, hypertension, tachycardia.
- Allergic: Acute hypersensitivity reactions including anaphylaxis, angioedema, dyspnea, pruritus, rash, and urticaria
- CNS Symptoms: Aggressive behavior; agitation; ataxia; coma; confusion; decreased consciousness; dysarthria; encephalopathy; mania; and psychosis, including auditory and visual hallucinations, seizures, tremors
- Eye: Visual abnormalities.
- Gastrointestinal: Diarrhea.
- Hepatobiliary Tract and Pancreas: Liver enzyme abnormalities, hepatitis.
- Renal: Renal failure, renal pain (may be associated with renal failure)
- Hematologic: Thrombocytopenia, aplastic anemia, leukocytoclastic vasculitis, TTP/HUS
- Skin: Erythema multiforme, rashes including photosensitivity, alopecia.
Valclovir Tablets (Generic Valacyclovir Hydrochloride) - Overdosage
Acute renal failure and neurological symptoms, including confusion, hallucinations, agitation, decreased consciousness and coma, have been reported in patients receiving overdoses of Valacyclovir tablets (Valclovir Tablets). Nausea and vomiting may also occur. Caution should be exercised to prevent inadvertent overdosing. Many of the reported cases involved renally impaired and elderly patients receiving repeated overdoses, due to lack of appropriate dosage reduction.
Patients should be observed closely for signs of toxicity.Precipitation of Acyclovir in renal tubules may occur when the solubility (2.5 mg/mL) is exceeded in the intratubular fluid. In the event of acute renal failure and anuria, the patient may benefit from hemodialysis until renal function is restored. Haemodialysis significantly enhances the removal of acyclovir from the blood and may, therefore, be considered a management option in the event of symptomatic overdose.
Valacyclovir Hydrochloride During Pregnancy
Valacyclovir Hydrochloride (Valclovir tablets) has been classified by the US FDA as Pregnancy Category B. Valclovir tablets should be used during pregnancy only if the potential benefit justifies the potential risk to the foetus. Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Valacyclovir Hydrochloride Nursing Considerations
