Uses of Generic Kenalog 40 mg (Kenacort Injection by Piramal Healthcare)
Generic Kenalog Injection 40 mg and 10 mg is indicated for intra-articular or intrasynovial administration, and for injections into tendon sheaths, as adjunctive therapy for short-term administration (to tide the patient over an acute episode or exacerbation) in: synovitis of osteoarthritis, rheumatoid arthritis, acute and subacute bursitis, acute gouty arthritis, epicondylitis, acute nonspecific tenosynovitis, post-traumatic osteoarthritis.

Kenalog injection is a Sterile Triamcinolone Acetonide Suspension USP suitable for intralesional and intra-articular injection. It is a synthetic glucocorticoid corticosteroid with marked anti-inflammatory action. Each milliliter of this injection contains 10 mg of Triamcinolone acetonide, with sodium chloride for isotonicity, 0.9% (w/v) benzyl alcohol as a preservative, 0.75% carboxymethylcellulose sodium, and 0.04% polysorbate 80; sodium hydroxide or hydrochloric acid may have been added to adjust pH between 5.0 and 7.5. Kenacort Injection (Triamcinolone Acetonide Injectable Suspension) - Dosage
The initial dose of Kenacort Injection may vary from 5 to 60 mg per day depending on the specific indication being treated. In situations of less severity, lower doses will be sufficient while in selected patients higher initial doses may be necessary.
Systemic:
The recommended starting dose is 60 mg, injected deeply into the gluteal muscle. Loss of fatty tissue located directly below the layer of skin may occur if the injection is not properly given. The Dosage is usually adjusted within the range of 40 mg to 80 mg, depending upon patient response and duration of relief. However, some patients can well controlled on doses as low as 20 mg or less.
Hay fever or pollen asthma:
Patients with hay fever or pollen asthma who are not responding to pollen administration and other conventional therapy may obtain a remission of symptoms lasting throughout the pollen season after a single injection of 40 mg to 100 mg. Treatment of acute exacerbations of multiple sclerosis:
Daily doses of 160 mg of Triamcinolone Acetonide injection for a week followed by 64 mg every other day for one month are usually recommended.

Intra-articular administration:
A single local injection of Triamcinolone Acetonide is normally sufficient, but several injections may be required for adequate relief of symptoms.
Initial dose:
2.5 mg to 5 mg for smaller joints and from 5 mg to 15 mg for larger joints, depending on the specific indications being treated. For adults, doses up to 10 mg for smaller areas and up to 40 mg for larger areas have usually been enough. Single injections into several joints, up to a total of 80 mg, have been administered.
How to Use Triamcinolone Acetonide Injection

Due to the potentiality for cutaneous atrophy, the site of injection and volume of injection should be carefully considered.
Keeping in mind that the greater the total volume employed the more corticosteroid becomes available for systemic absorption and systemic effects, many sites separated by one centimeter or more may be injected. When the corticosteroid is administered to provide a localized concentration, the lower dosages in the initial dosage range of Triamcinolone acetonde may produce the desired effect. The site and volume of the injection should be carefully considered while injecting the medication.
Triamcinolone Acetonide Injection Overdosage
The symptoms of chronic glucocorticoid overdose can include confusion, anxiety, depression, gastrointestinal cramping or bleeding, ecchymois, moon face and hypertension. After long-term use of Kenacort injection, rapid withdrawal can result in acute adrenal insufficiency (which also may occur in times of stress). Cushingoid changes can result from continued use of large doses of this medicine.
There is no specific treatment for acute overdose, but supportive therapy should be started and, should gastrointestinal bleeding take place, it should be managed.
Kenacort 40 mg Injection Side effects
The possible side effects of Kenacort 40 mg injection (Triamcinolone Acetonide Injection) which are associated with cardiovascular system are are bradycardia, cardiac arrest, cardiac arrhythmias, cardiac enlargement, circulatory collapse, congestive heart failure, fat embolism, hypertension, hypertrophic cardiomyopathy in premature infants, myocardial rupture following recent myocardial infarction.
The dermatologic side effects of Kenacort injection are acne, allergic dermatitis, cutaneous and subcutaneous atrophy, dry scaly skin, ecchymoses and petechiae, edema, erythema, hyperpigmentation, hypopigmentation, impaired wound healing, increased sweating, lupus erythematosus-like lesions, purpura, rash, sterile abscess, striae, suppressed reactions to skin tests, thin fragile skin, thinning scalp hair, urticaria.
The endocrine side effects of Triamcinolone Acetonide Injection include decreased carbohydrate and glucose tolerance, development of cushingoid state, glycosuria, hirsutism, hypertrichosis, increased requirements for insulin or oral hypoglycemic agents in diabetes, manifestations of latent diabetes mellitus, menstrual irregularities, secondary adrenocortical and pituitary unresponsiveness.
The possible allergic reactions are Anaphylaxis including death and angioedema.
The fluid and electrolyte disturbances include congestive heart failure in susceptible patients, fluid retention, hypokalemic alkalosis, potassium loss and sodium retention.
The gastrointestinal side effects are Abdominal distention, bowel/bladder dysfunction, elevation in serum liver enzyme levels, hepatomegaly, increased appetite, nausea, pancreatitis, peptic ulcer with possible perforation and hemorrhage, perforation of the small and large intestine specially in patients with inflammatory bowel disease and ulcerative esophagitis.
The possible Metabolic side effects include Negative nitrogen balance due to protein catabolism. The possible Musculoskeletal adverse effects include Aseptic necrosis of femoral and humeral heads, calcinosis (following intra-articular or intralesional use), Charcot-like arthropathy, loss of muscle mass, muscle weakness, osteoporosis, patholog ic fracture of long bones, post injection flare (following intra-articular use), steroid myopathy, tendon rupture and vertebral compression fractures.
The Neurologic/Psychiatric side effects that may occur include Convulsions, depression, emotional instability, euphoria, headache, increased intracranial pressure with papilledema (pseudotumo cerebri) usually following discontinuation of treatment, insomnia, mood swings, neuritis, neuropathy, paresthesia, personality changes, psychiatric disorders and vertigo.
The possible Ophthalmic side effects are Exophthalmos, glaucoma, increased intraocular pressure, posterior subcapsular cataracts, rare instances of blindness associated with periocular injection.

Warnings & Precautions When Using Triamcinolone Acetonide Injection

Serious neurologic events, some even resulting in death, have been reported with epidural injection of corticosteroids including Kenacort 40 mg. Specific events reported include, but are not limited to, spinal cord infarction, paraplegia, quadriplegia, cortical blindness, and stroke.
Exposure to excessive quantities of benzyl alcohol has been associated with toxicity (hypotension, metabolic acidosis), particularly in neonates, and an increased incidence of kernicterus, specially in small preterm infants. Rare instances of anaphylaxis have occurred in patients receiving corticosteroid therapy .
Corticosteroids can produce reversible hypothalamic-pituitary-adrenal (HPA) axis suppression with the potential for glucocorticosteroid insufficiency after withdrawal of treatment.
Corticosteroids may exacerbate systemic fungal infections and hence should not be used in the presence of such infections unless they are required to control drug reactions.
Kenacort 40 mg, like many other steroid formulations, is sensitive to heat. Hence, this medication should not be autoclaved when it is desirable to sterilize the exterior of the vial.
The lowest possible dose of corticosteroid should be used to control the condition under treatment. When reduction in dosage can be done, the reduction should be gradual.
Triamcinolone Acetonide Injection During Pregnancy
Triamcinolone Acetonide Injection has been classified by the US FDA as Pregnancy Category C.
Corticosteroids have been proven to be teratogenic in many species when given in doses equivalent to the human dose. Animal studies in which corticosteroids have been administered to pregnant mice, rats, and rabbits have yielded an increased incidence of cleft palate in the offspring. There are no adequate and well-controlled studies conducted in pregnant women. Corticosteroids should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Infants born to mothers who have received corticosteroids during pregnancy should be carefully observed for signs and symptoms of hypoadrenalism.

Triamcinolone Acetonide Injection Nursing Considerations
Systemically administered corticosteroids including Triamcinolone Acetonide appear in human milk and could suppress growth, interfere with endogenous corticosteroid production, or cause other untoward effects. Caution should be exercised when corticosteroids are administered to a nursing woman.
Do not use Kenacort 40 mg without telling your doctor if you are breast-feeding a baby.
