Generic Altace (Ramipril) is an angiotensin converting enzyme (ACE) inhibitor which is used to treat hypertension inorder to lower blood pressure. Lowering blood pressure will reduce the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions. Ziram tablets may be used alone or in combination with thiazide diuretics. Ziram tablets, the Generic eqivalent of Altace are manufactured by FDC Limited in the strengths of 2.5 mg and 5 mg.

The active ingredient present in Ziram Tablets is Ramipril. Each tablet of Ziram contains 2.5 mg or 5 mg of Ramipril.

Uses of Generic Altace (Ziram by FDC)

This medicine is used to treat high blood pressure (hypertension) or congestive heart failure, and to improve survival after a heart attack. It belongs to a group of drugs called ACE inhibitors (Angiotensin Converting Enzyme).

Ramipril Tablets - Dosage

The dosage of Generic Altace tablets (Ramipril) for various indications is given below:

Treatment of Hypertension:
The starting dose of Ramipril tablets is 2.5 mg to 20 mg once daily. Adjust dosage according to blood pressure response after 2 to 4 weeks of treatment. The usual maintenance dose following titration is 2.5 mg to 20 mg daily as a single dose or equally divided doses
Reduction in the risk of myocardial infarction, stroke, or death from cardiovascular causes:
The starting dose is 2.5 mg once daily for 1 week, 5 mg once daily for 3 weeks, and increased as tolerated to a maintenance dose of 10 mg once daily.
Heart failure post-myocardial infarction:
The starting dose is 2.5 mg twice daily. If patient becomes hypotensive at this dose, decrease the dosage to 1.25 mg twice daily. Increase dose as tolerated toward a target dose of 5 mg twice daily, with dosage increases about 3 weeks apart.

How to Use Generic Altace (Ziram Tablets)

Generic Altace (Ramipril) is used to treat hypertension and lower blood pressure so as to reduce the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions. Take Generic Altace tablets exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Your doctor may occasionally change your dose to make sure you get the best results. It can be taken with or without food. Swallow the medicine whole.
You may open the capsule and sprinkle the medicine into a half-cup (4 ounces) of water, apple juice, or applesauce to make swallowing easier. Swallow without chewing. You may store the mixture for up to 24 hours at room temperature or up to 48 hours in a refrigerator.
Conditions that may cause very low blood pressure include: vomiting, diarrhea, heavy sweating, dehydration, a low salt diet, or taking diuretics (water pills). Tell your doctor if you have a prolonged illness that causes diarrhea or vomiting. Your blood pressure will need to be checked often. Your kidney or liver function may also need to be tested. Visit your doctor regularly.
If you need surgery, tell the surgeon ahead of time that you are using this medicine. You may need to stop using the medicine for a short time. If you are being treated for high blood pressure, keep using it even if you feel fine. High blood pressure often has no symptoms.
Take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

Ramipril (Ziram Tablets) Contraindications

Ziram tablets are contraindicated in patients with a hypersensitivity to Ramipril or any inactive ingredient present in Ziram tablets.Ramipril is also contraindicated in cases of Angioedema related to previous treatment with an ACE inhibitor, or a history of hereditary or idiopathic angioedema. Do not co-administer aliskiren with Generic Altace (Ramipril tablets) in patients with diabetes.

Ramipril Side effects

The most common side effects of Ramipril tablets (Generic Altace) in patients undergoing treatment for hypertension are headache, dizziness, fatigue, and cough. Get emergency medical help if you have any signs of an allergic reaction including: hives, severe stomach pain, difficulty breathing, and swelling of your face, lips, tongue, or throat.

Stop taking Generic Altace (Ziram tablets) and call your doctor at once if you have a serious side effect such as:
• feeling like you might pass out
• high potassium level (slow heart rate, weak pulse, muscle weakness, tingly feeling
• dry mouth, thirst, confusion, swelling, and urinating less than usual or not at all
• pale skin, dark colored urine, easy bruising or bleeding
• jaundice (yellowing of the skin or eyes)
• fever, chills, body aches, flu symptoms, sores in your mouth and throat

Less serious side effects may include:
• cough
• tired feeling, headache
• dizziness, spinning sensation
• nausea, vomiting

Overdose symptoms may include feeling extremely dizzy or light-headed, or fainting. This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects.

Warnings & Precautions When Using Ramipril tablets (Generic Altace)

You should not use Ramipril tablets if you are allergic to ramipril or to any other ACE inhibitor, such as benazepril (Lotensin®), captopril (Capoten®), fosinopril (Monopril®), enalapril (Vasotec®), lisinopril (Prinivil®, Zestril®), moexipril (Univasc®), perindopril (Aceon®), quinapril (Accupril®), or trandolapril (Mavik®).

To make sure you can safely take this medicine, tell your doctor if you have any of these other conditions:
• kidney disease (or if you are on dialysis)
• liver disease
• diabetes
• a connective tissue disease such as Marfan syndrome, Sjogren's syndrome, lupus, scleroderma, or rheumatoid arthritis
• if you are also taking telmisartan (Micardis ®)
• if you have ever had a severe allergic reaction

Avoid drinking alcohol. It can further lower your blood pressure and may increase some of the side effects of this medicine. Do not use salt substitutes or potassium supplements while taking it unless your doctor has told you to. Avoid getting up too fast from a sitting or lying position, or you may feel dizzy. Get up slowly and steady yourself to prevent a fall.

Ramipril During Pregnancy

Ramipril has been classified by the US FDA as Pregnancy Category D. Use of drugs such as Ramipril that act on the renin-angiotensin system during the second and third trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death. This medicine can cause severe harm to an unborn baby. Do not use Generic Altace without your doctor's consent if you are pregnant. Stop using this medication and tell your doctor right away if you become pregnant during treatment.

Ramipril During Breastfeeding

It is not known whether Ramipril passes into breast milk or if it could harm a nursing baby. Administration of a single 10 mg oral dose of Generic Altace resulted in undetectable amounts of Ramipril and its metabolites in breast milk. However, because multiple doses may produce low milk concentrations that are not predictable from a single dose, do not use Ramipril tablets in nursing mothers. Do not use Generic Altace tablets without telling your doctor if you are breastfeeding a baby.
